Boswellia and joint pain: human evidence reviewed
Meta-analyses of randomised trials in osteoarthritis report that Boswellia serrata extracts reduce pain and stiffness scores compared with control, and the results are more consistently positive than for many joint supplements. However, the trials are small, use different standardised extracts, and many are linked to extract manufacturers, so the size of the effect is uncertain and long-term safety data are limited.

What the results show
The direction of effect is consistent across meta-analyses: Boswellia extracts outperform placebo on pain and stiffness scores. The magnitude is harder to trust. Many trials are small, use proprietary extracts standardised to different boswellic acids, and are sponsored by or linked to extract manufacturers. Very large standardised effect sizes in small trials are a common sign of overestimation, which is why independent multi-centre trials would be needed to confirm how much benefit a typical patient can expect.
What the research does not prove
- That Boswellia rebuilds cartilage or halts osteoarthritis progression.
- That one standardised extract’s results apply to generic frankincense powder.
- That joint blends containing Boswellia at undisclosed doses work.
- That it is proven for rheumatoid arthritis, where evidence is sparser.
Safety
- Short-term trials report good tolerability, with occasional digestive upset.
- Reviews note possible interactions via liver drug-metabolising enzymes; check with a pharmacist if you take regular medicines.
- Rare liver signals have been reported; stop if symptoms of liver problems develop.
- Safety in pregnancy and breastfeeding is not established.
Limitations of the evidence base
- Small sample sizes and short durations.
- Heterogeneous extracts and standardisation.
- Manufacturer involvement in much of the research.
- Mixed comparators (placebo and active drugs) in pooled analyses.
